Addition of Members in Ration Card |
Addition of an adult unit is made on request on application form duly attested. The applicant is required
to submit the deletion/surrender certificate obtained from the rationing authority
from where the unit has migrated and two passport size group family photographs
along with the form. Children below the age of 3 years are added on production of
a photocopy of the birth certificate, two photographs along with the application form.
REQUIREMENT FOR ADDITION OF MEMBERS
1. Application form duly attested
2. Deletion/surrender certificate obtained from the rationing authority from where the unit has migrated, in case of addition of adult unit.
3. Two passport size group family photographs.
4. Incase of addition of name of Child less than 5 years, Birth Certificate and Teekaran Copy must be submitted.
5. Incase of addition of name of Child greater than 5 years, Birth Certificate and Affidavit must be submitted.
